This paper explains the "Intake Classific" and "CLUSTER" fields present in the Excel file "Illness Classificatoin SF36 MFI and Symptoms.xls" (note the "toin" not "tion" in the filename).

Without this paper one could only guess the meaning of values present in these fields.
